MyChart is a free online tool to access your medical record at Mackenzie Health from your personal device (phone, computer, tablet etc.). Log on to any computer, from anywhere, at any time, to stay on top of your health information including test results, appointments, summaries and medications. E-Check In with MyChart |
E-Check In is available to patients with a MyChart account and combines online pre-registration and check-in to get you to your appointment faster for outpatient clinics at Mackenzie Health. Upon arrival on the day of an appointment all patients register by visiting Patient Registration, located on the second floor of C-Wing, or designated Registration Area. Patients who have completed E-Check In will have fewer questions to answer or information to complete, getting them to their appointment faster. You will receive an E-Check In notification to confirm and update registration information (address, phone number, emergency contact, family physician, health card version code/expiry date) seven days prior to your appointment. E-Check In can be completed up to the time of your appointment. However, if you would like to avoid a reminder phone or text message, complete E-Check In before 72 hours prior to your scheduled visit. If you wish to change any patient demographic information such as your name, date of birth or address, please contact our Patient Registration department, by calling 905-883-1212 ext. 2004 (local to Richmond Hill) or 905-832-4554 ext. 2004 (local to Vaughan) to make these changes. |
MyChart Security |
We take great care in ensuring your personal health information is private and secure. MyChart is secured with similar encryption technologies as those used by major banks for online banking. For your security, you create and control your MyChart password. |
Proxy Access for another Patients' Record |
If you are the parent or legal guardian of a child under the age of 16, or if you would like access to an adult patient's MyChart, you can request proxy access to their MyChart account. If you would like to set up proxy access, please submit your completed MyChart Proxy Consent Form to Health Information Services. You can send your completed form via e-mail to mychart@mackenziehealth.ca, mail, fax it to 905-883-2141 or by drop it off in person to the Health Information Services department.
