Complex Continuing Care
At Mackenzie Health we offer many Complex Continuing Care, Rehabilitation, Stroke, and Palliative Care services as part of our Chronic Disease and Seniors Health Centre of Excellence.
We provide enhanced quality of life by maximizing strengths and abilities, and recognizing individual needs for all patients and their families.
The goal is to stabilize and transition patients to enable them to be transferred to appropriate level of care.
It is the belief of the care team that each patient and their family should play an integral role in designing the best care options to meet the unique challenges of reintegration to the community.
Patients in Complex Continuing Care are generally adults 19 years of age or older and are medically complex with high intensity care requirements.

Co-payment will apply to all patients as per Ontario Health guidelines.
