The MackenzieHelps Program is a wrap around community care model that supports patients transitioning from hospital to home. There are two streams in the program:
- Mackenzie Helps Regular: supports individuals for 16-weeks through interdisciplinary home services and transitional support in the community.
- Mackenzie Helps Plus: supports individuals for 26-weeks and provides the addition of behavioural supports
This model ensures patients are safely transitioning home while receiving a range of supports they need for a full recovery.
The MackenzieHelps team consists of three partners in your care: SE Health, CHATS and LOFT Behavioural Support Services.
Before you leave the hospital, your MackenzieHelps Navigator meets with you, your family, your hospital team and your MackenzieHelps team to create your care plan.
This plan is shared with everyone involved in providing your care after you leave the hospital. Your first home visit will be scheduled before you leave the hospital.
Your MackenzieHelps team will:
- Visit you within 24 hours of you leaving the hospital
- Check-in with you every day for the first week at your preferred time –after the first week, you and your team will decide together how often you require a check-in
- Work closely with the hospital to ensure your goals are being met once you’re home
- Keep your primary care provider (family doctor or nurse practitioner) up to date on your progress
- Provide psychogeriatric support
- Provide in home therapeutic recreation
- Use different ways to check-in and care for you, such as:
- Home visits
- Phone calls
- Virtual Care
- Coordinate other services for you including:
- Meals on Wheels
- Transportation services
- Caregiver support programs
- Adult day program
MackenzieHelps Plus offers Behavioural Support Services including:
- Provide Behavioural support Services including:
- Behavioural assessments
- Behavioural support plan
- Capacity building, training and education
- Meaningful Engagement / Respite services
If your care needs change, so will your care plan. You may need more services at times, or you may need less. MackenzieHelps Plus was designed with this flexibility in mind.
These supports are there so you have what you need to be at home.

If you do not have a caregiver or your caregiver cannot take you home on the day of discharge, MackenzieHelps will drive you home and will help you get settled. We can assist you in getting your prescription from your pharmacy.
If your medical condition changes and you need hospital care, MackenzieHelps will continue to support you once you’re at home again. Your MackenzieHelps team will be kept informed so they can plan your transition back home.
