Q: What is the Mackenzie Health 50/50 Lottery?
A: The Mackenzie Health 50/50 lottery is a monthly, online community lottery designed to raise funds for Mackenzie Health’s highest priority needs. To learn more about Mackenzie Health Foundation please visit mackenziehealth.ca/foundation.

 

Q: Can I purchase tickets in person, or are they only available online?
A: Tickets can only be purchased online at MHF5050.ca. Tickets cannot be purchased in-person.

Q: I don’t have a computer, can I purchase tickets by telephone?
A: Unfortunately, we are not selling tickets by telephone at this time. Tickets can only be purchased online at MHF5050. If you require assistance, please call 1-833-959-0933 and someone will be happy to assist you.

Q: How do I get my ticket when I order online?
A: Purchased tickets are delivered to the e-mail address supplied at time of purchase. Please allow 12 hours for the e-mail to be sent. Please remember to check your spam and junk, as some e-mail services may incorrectly flag your incoming tickets. Tickets are delivered as "Mackenzie Health 5050" from noreply@ascendfs.com. If you have not received your tickets after 12 hours, please e-mail 5050@mackenziehealth.ca.

Q: Which payment methods are accepted?
A: MasterCard, Visa, and American Express will be accepted. Regrettably, we do not accept debit cards, cheque, money orders, or bank and/or electronic transfers.
Note: The transaction will appear on your credit card bill as "MACKENZIE HEALTH FOUND"

Q: I’m blocked from purchasing tickets. What can I do?
A: In order to purchase tickets online, the website is required to know your location. You must be located within the Province of Ontario, Canada at the time of purchase. If you are unable to access the site it may be due to one of the following reasons:

  • You declined the website request to access your location
  • Your device’s location settings are permanently disabled
  • You are located outside of the Province of Ontario, Canada and unfortunately are not able to purchase tickets

Please ensure the following:

  • Your device settings have Location Services enabled
  • You allow the website to access your location
  • If your issue persists, try closing the website, clear your browser caches, and try visiting the site again

Q: Can I buy more than one ticket?
A: Yes, you can purchase up to 10 sets of tickets (i.e., $50 for 150 tickets x 10) Each set will appear as a separate transaction on your credit card statement. In your confirmation email, you will receive one PDF attachment for each bundle purchased (i.e., $50 for 150 tickets, $20 for 30 tickets, $10 for 5 tickets).

Q: Do my tickets enter me into EVERY monthly draw?
A: No. Tickets purchased for the current Mackenzie Health 50/50 draw are only eligible for the current draw and do not gain you entry into future Mackenzie Health 50/50 draws. There will be a new Mackenzie Health 50/50 lottery every month starting after the initial draw on November 1, 2021. The jackpot resets for each monthly draw, and ticket sales start anew.

Q: Can I purchase tickets as a gift for someone else or as a group?
A: Yes, this is possible. Note that some credit card companies require the Billing Address to match what they have on file, so this may not work for everyone.

Please follow these steps to purchase the tickets in someone else's name or as a group:
In Step 1:

  • Choose the number of tickets you'd like to purchase.

In Step 2:

  • Enter the name and address of the person you would like to gift the tickets to, or;
  • If it's a group ticket, use the group leader's name and add "in trust" after the last name. (Only one person's name can be on the ticket.)
  • You can either enter your email address if you want to hold the ticket until it's time to give it to them, or;
  • email it directly to them by typing their email address into the field.

In Step 3:

  • Enter your own info as the purchaser.

Q: Will I receive a tax receipt for my purchase?
A: Tax receipts are not issued for participation in the 50/50 raffle. Tickets purchased for lotteries and games of chance are not tax deductible in accordance with Canada Revenue Agency policies.

Q: I can’t access the site, what do I do?

Q: I didn’t receive my tickets, what do I do?
A: First, please check your junk mail and spam folders as tickets can sometimes be filtered due to strict security filters. Tickets are delivered as "Mackenzie Health 5050" from noreply@ascendfs.com. Whitelist this address with your email provider. If you check spam and still can't find your tickets, please contact 5050@mackenziehealth.ca with:

  • Your name
  • The name under which the tickets were purchased
  • The date the tickets were purchased
  • The email address associated with the ticket purchase

Q: My transaction did not process, what do I do?/ A message saying "Error 4" appeared when I tried to purchase.
A: Error 4 occurs when payment is not able to go through on your credit card or your credit card company has blocked the payment. Please ensure that the credit card information you entered is correct, that the card has been activated and is not expired, and that the Billing Address matches what your credit card company has on file. If all of this information is correct but the error still occurs, please contact your credit card company

Q: When and how will the winners be announced?
A: Any winning numbers (for either Early Bird draw or the Grand Prize draw) will be posted on MHF5050.ca immediately following the respective draw. Announcements will also be made by email to all lottery ticket buyers.

Q: Will you notify me if I win?
A: Should your ticket purchase(s) be associated with any winning number(s), we will attempt to contact you using the details you provided at the time of purchase.

Q: Do I need my ticket to claim the prize?
A: Yes. You will have a digital ticket in your e-mail that you must present to claim the prize. Upon presenting a winning 50/50 ticket, the Winner must: (i) Provide a photo ID proving their age to be over 18; (ii) Completely fill out and sign the 50/50 Lottery Prize Claim Form issued by a Mackenzie Health Foundation staff member (iii) Sign their name on the Winning Ticket Once the certified Winner is verified, they will be paid by Electronic Fund Transfer (EFT) for the amount posted at the event for which they bought their ticket. Winners must have the original winning ticket, and no copies. If the Winner cannot provide photo ID, then the winning proceeds will be held until such identification can be provided. EFT processing will commence after certification is finalized and processing times vary and can take up to two weeks.

Q: How long does a winner have to claim their prize?
A: All prizes must be claimed within six months of the relevant draw, unless otherwise specified by Mackenzie Health Foundation. Thereafter any unclaimed prizes shall be donated to a charity/non-profit organization approved by the Alcohol and Gaming Commission of Ontario (AGCO).

Q: How is this different from the 50/50 Staff Lottery? / Can I buy tickets for the 50/50 Staff Lottery and the 50/50 Community Lottery?
A: The 50/50 Staff Lottery is only open to eligible staff of Mackenzie Health but the 50/50 Community Lottery is open to all individuals in Ontario who are 18 years or older. Mackenzie Health physicians, staff and volunteers can participate in both lotteries! However, unlike the Staff Lottery, you will need to purchase tickets online for every monthly draw you wish to participate in. Your ticket purchases do not carry over month to month.

Q: I work for Mackenzie Health – am I still eligible to buy tickets for the 50/50 Community Lottery?
a) Yes, eligible Mackenzie Health staff are encouraged to participate in the 50/50 Community Lottery.

Please note the following parties cannot participate:
b) Members of Mackenzie Health and Mackenzie Health Foundation’s Board of Directors; as well as their family members living in the same household
c) Mackenzie Health Foundation’s employees, including those who are on paid or unpaid leave; as well as their family members living in the same household
d) Senior Leadership Team of Mackenzie Health Foundation and Mackenize Health and independent contractors who fill these roles; as well as their family members living in the same household
e) Employees of Ascend Fundraising Solutions
f) Persons under the age of 18 years
