Virtual assessments connecting patients and health care providers during COVID-19

The COVID-19 pandemic has posed many challenges for the care of hospitalized patients. At Mackenzie Health, we've implemented an innovative way to support the transition of hospitalized patients to the community through the use of virtual visits.

After suffering a fall at home, 78 year-old Janet, a patient at Mackenzie Richmond Hill Hospital, knew she would need to be assessed before leaving hospital to enter a retirement home. With restrictions limiting who could enter the hospital due to COVID-19, an in-person assessment by retirement home staff would be impossible. But thanks to the innovative work of health care professionals at Mackenzie Health, Janet was virtually assessed so she could safely leave hospital for her new home.

"It was comforting to know that the staff wanted to help me so much," said Janet.

As part of the assessment, retirement home staff were able to observe Janet in her hospital room and assess her mobility, collaborate with her care team to assess her level of recovery and begin the process of her transition.
