Freedom of Information

The Freedom of Information and Protection of Privacy Act (FIPPA) governs access to information and privacy.

The Freedom of Information and Protection of Privacy Act (FIPPA) is an Ontario statute that was enacted in 1990 to govern access to information and privacy. In December 2010, the Ontario government enacted the Broader Public Sector Accountability Act, which extended the application of FIPPA to Ontario hospitals as of January 1, 2012, in order to demonstrate sector’s deep and continuing commitment to transparency and accountability.

  1. Access to Information – to provide the public a right of access to hospital records subject to limited exemptions;  and
  2. Privacy - to protect the privacy of individuals with regards to their personal information and to provide a right of access to their personal information held by the hospital.

Mackenzie Health proactively discloses information that we make routinely available through our website. Such information is continually updated. Some records such as executive contracts, accreditation reports, annual reports and infection control indicators are available on our website and may be accessed on our performance page.

 

Members of the public can request access to hospital records by making a Freedom of Information (FOI) request. Types of records the hospital maintains relate to hospital budgets, minutes, agendas, and other records including policies and procedures.

If you would like to initiate a formal request for hospital records, please complete and submit an FOI Request Form along with a $5.00 application fee to Mackenzie Health.

You may submit your completed form via mail to the address below or via email. Your request will be initiated with receipt of payment, as outlined on the form and will be processed in accordance with FIPPA.

Additional fees may apply depending on the nature of the request submitted. The application fee is mandatory and cannot be waived as per FIPPA.

Please note that access to your own personal health information is governed by the Personal Health Information Protection Act, 2004 (PHIPA). For access to your own personal health information visit MyChart or contact Health Information Services.

The Directory of Records (DOR) provides general descriptions of records covered by the Freedom of Information and Protection of Privacy Act (FIPPA).

Personal Information Banks

Mackenzie Health is required under the Freedom of Information and Protection of Privacy Act (“FIPPA”) to produce an index of Personal Information Banks (PIB) – a description of the collections of personal information (recorded information about an identifiable individual) maintained to support our programs and activities, where such collections are organized and retrievable by an individual’s name or another identifier.
