Nurse-Led Outreach Team
The Nurse Led Outreach Team (NLOT) provides specialized, time sensitive care to seniors in Long Term Care Homes (LTCHs).
- Preventing avoidable transfers from Long-term care (LTC) homes to Emergency Department
- Facilitate timely, seamless, and sustainable discharge from the hospital for hospitalized LTC residents.
- Mackenzie Health NLOT is a team of nurses, NPs and RNs who travel to LTC homes and support 10 LTCHs in the Mackenzie Health catchment area.
Nurse Practitioners provide advanced assessment diagnosis, treatment, and management of complex medical conditions and injuries, order a range of diagnostic tests, prescribe medications, and can intervene and treat acute and chronic medical conditions.
- Long Term Care residents who currently reside in Long Term Care home within Mackenzie health catchment area.
- Recent change in condition or complex clinical situation
NLOT services are available to all residents and their families, based at the 10 Long Term Care Homes.
- No written referrals are needed to access the NLOT team services.
- Long Term Care Home and hospital staff can contact NLOT NPs via phone or email when needed.
- Long Term Care residents and their Power of Attorney substitute decision makers’ may contact NLOT via phone.
- The following information should be included when request NLOT assessment:
- Name of the Long Term Care home
- Name and room number of the Long Term Care resident
- Reason for referral
- Health/ Clinical issue to address
