Patient Registration

This page will provide you with all of the information you need in order to register for your visit to the hospital.

At Mackenzie Health, we strive to provide our patients with the best health care experience. To help reduce the time that it will take you to register when you come to the hospital, ensure that your registration information is up to date and get to your appointment faster, we offer eCheck-In for all outpatient clinics:

  • Online pre-registration through My Chart and
  • On-site check in through our Self-Serve Patient Registration Kiosk

You can log on through MyChart to verify your registration information at any time and check in for your appointment seven days prior. Please view our MyChart page to get started.

You can easily sign up for MyChart and access our eCheck-in service in one of the following three ways:

  1. When your appointment is being scheduled, you will be asked for your e-mail address and an 'Activation Code' will be sent to you with instructions on how to create your MyChart account.  If you are not asked about signing up, simply indicate you are interested in MyChart with the scheduler or during registration at an upcoming appointment.  Note that Activation Codes are only valid for 30 days.
  2. At discharge from Mackenzie Health, you will be provided an After Visit Summary which contains a MyChart ‘Activation Code’ at the bottom of the first page.  Click the link below to sign up using your Activation Code.
  3. Former patients can self-sign up on the MyChart sign up page

Log into your MyChart account and under the To Do section on the home page, select the upcoming appointment you would like to eCheck-In for and follow the prompts.  Once you have completed eCheck-In, you will be provided a barcode that you can print out or pull up on your mobile device and scan at the Self-Serve Patient Registration Kiosk when you arrive for your appointment to expedite visit.

You can complete eCheck-In up to seven (7) days prior to your appointment.  Here you can verify your registration information and complete any associated pre-visit questionnaires.

MyChart will send an automated notification two (2) days prior to the appointment date to remind you of your appointment.

If you are not able to eCheck-In online prior to your appointment, a clerk will call you to complete the pre-registration process over the phone.  

When you arrive to Mackenzie Health, please check in using the Self-Serve Patient Registration Kiosks. At Mackenzie Richmond Hill Hospital, Patient Registration is located in the C-Wing, Level 2. At Cortellucci Vaughan Hospital, Patient Registration is at near the Vic De Zen Family Welcome Centre on Level 1. The kiosks will prompt you to verify your information and you will be able to edit specific demographic information yourself. If needed, you may be directed by the kiosk to a Patient Assistance Area where our staff will be happy to update your information with you.

*For patients scheduled in our Chronic Kidney Disease clinics, please check in at the front desk upon arrival.  For patients scheduled in our Mental Health clinics, please check in using the kiosks located within the clinics.

If you are not able to E-Check In online prior to your appointment, please check in using the Self- Serve Patient Registration Kiosks located in the C Wing, Level 2. The kiosks will prompt you to verify the same information and you will be able to edit specific demographic information yourself. If needed, you may be directed by the kiosk to a Patient Assistance Area where our staff will be happy to update your information with you.

When you arrive at Mackenzie Health, you will need to check in at Patient Registration.

Upon arrival to Patient Registration, please follow the prompts at the Self-Serve Patient Registration Kiosks.  Note that you will have been provided a barcode at completion of eCheck-In that you can print out or pull up on your mobile device that you can scan at the kiosks to expedite your check in time.
