Recognizing that exemplary patient care includes listening and learning from others and incorporating new ideas and perspectives to continually improve care, Mackenzie Health is seeking patients, family members and/or caregivers to join a new Patient and Family Advisory Council (PFAC). This new Council will provide a formal and structured forum to encourage reflective feedback and elicit suggestions for improvement from our community on the care and services at Mackenzie Health.
Mackenzie Health has many opportunities to receive feedback from care recipients on its services and programs. These opportunities include daily rounding by nursing unit managers with patients and families, follow-up phone calls to discharged patients, various surveys, the patient relations office that receives feedback on behalf of the organization, community engagement sessions, the President’s Community Advisory Council and others.
The new PFAC builds on the many existing opportunities the organization has to gain feedback from patients, families and caregivers and will establish an ongoing partnership between care recipients and Mackenzie Health. By working together, this partnership will help Mackenzie Health to continually improve the care experience and identify additional improvement opportunities.
PFAC members will become trusted volunteer advisers to the organization to help embed the voice of patients into all aspects of care including actions, decisions and planning. Advisers will be able to reflect on their own experiences with Mackenzie Health and offer meaningful and actionable suggestions on how the organization could improve the patient experience at the Mackenzie Richmond Hill Hospital or one of its many community care locations. The improvements in the care experience will also benefit patients and families who experience care at the future Mackenzie Vaughan Hospital.
Through its commitment to incorporating the patient voice into all aspects of care Mackenzie Health will be better able to respond to the complex needs of individuals and their families, both medically and emotionally, as well as provide optimal guidance as patients transition back to the community after hospital care.

How to apply to become a PFAC Advisor?
Mackenzie Health invites patients, family members and/or caregivers of patients who have received care within the past two years to apply to become a PFAC Advisor by completing an on-line application. The application, as well as more information, is available on Mackenzie Health’s website.
Applications are being accepted until Monday, September 8, 2014. Applicants will be contacted to arrange an interview to determine eligibility and confirm ongoing availability. The first meeting will take place in September/October 2014.
