Complex Continuing Care

At Mackenzie Health we offer many Complex Continuing Care, Rehabilitation, Stroke, Palliative Care and Adult Day Services as part of our Chronic Disease and Seniors Health Centre of Excellence. These services are provided in A-Wing at Mackenzie Richmond Hill Hospital and in the community where appropriate.

The goal is to stabilize and transition patients to enable them to be transferred to appropriate level of care. Respite care or caregiver relief is available.

We provide enhanced quality of life by maximizing the strengths and abilities and recognizing individual needs for all patients and their families. It is the belief of the care team that each patient and their family should play in integral role in designing the best care options to meet the unique challenges of reintegration to the community.

Patients in Complex Continuing Care are generally adults 19 years of age and older, are medically stable and have had any infection control issues identified prior admission.

Co-payment

Applies to all patients that will be discharged to Long Term Care facility or free standing Continuing Care Centre, or will remain in Continuing Care Centre for long period of time (as per Ministry guidelines).

A co-payment fee may apply if the physician feels that the patients' needs may be met in another setting. This is reviewed on a weekly basis. The co-payment fee is geared to income up to a maximum of $59.82 per day. Co-payment is calculated based on the patients' previous years income tax return/notice of assessment.
